At Next Health, the longevity clinic that pioneered whole-body optimization, therapeutic plasma exchange is delivered with the rigor it deserves: a physician-designed protocol, performed by our registered nurses, inside a full longevity ecosystem built around it. We remove a portion of your plasma, along with the substances it carries, replace it with sterile albumin, and return your own cells to you.
TPE is a clinically established procedure. Its use for longevity and performance is off-label and investigational; it is not a treatment or cure for any condition. Candidacy is determined by a physician.
Right now your plasma is loaded. The amber haze illustrates the substances that accumulate in plasma over time.
An illustrative visualization of the plasma exchange, not a depiction of your individual blood.
Over time, various proteins and metabolic byproducts accumulate in plasma, and research into how this relates to aging is ongoing. A standard physical does not measure many of these markers, so baseline testing gives you a fuller picture of where you stand. Before treatment, a baseline test shows your current biomarker levels and confirms you’re cleared for the procedure. Therapeutic plasma exchange then removes a portion of your plasma and replaces it with sterile, purified albumin.
We separate and draw off a portion of your plasma, along with the substances it carries.
In its place, sterile purified albumin: a clean, pristine medium for your cells to function in.
Your own red cells, white cells and platelets are returned to you alongside the fresh replacement fluid.
Most of what shapes how you age never shows up on a routine panel. If the profile below sounds like you, this is the layer worth measuring next.
Your blood never leaves a sealed sterile circuit. Click through to see exactly what happens, and why "blood outside my body" is nothing to fear.
You sit down with a clinical or longevity provider to learn how TPE works, get every question answered, and decide together whether it’s the right fit for your goals and health history. No pressure, no obligation.
Each session takes roughly 2–3 hours · physician-designed protocol · performed by a registered nurse throughout.
These are the recommended add-ons that power up your TPE session — complementary therapies our team suggests to get more from the time you're already here.
NAD+ is a coenzyme involved in cellular energy metabolism. Many patients add an NAD+ infusion during their session. Your provider will discuss whether it fits your goals.
Breathing oxygen in a pressurized chamber raises the amount of oxygen your blood can carry to your tissues. Pairing it with your TPE visit is a favorite of patients optimizing for recovery and performance. Your provider will advise whether it fits your goals.
Self-pay. Some patients use HSA or FSA funds; eligibility depends on your plan, so we recommend confirming with your plan administrator. Baseline biomarker testing, which confirms your levels and clears you for treatment, is available at additional cost. Each path begins with the same private consultation.

Public figures shown have engaged with Therapeutic Plasma Exchange at Next Health. Individual experiences vary.
Plasmapheresis is the broad term for separating plasma from blood. Therapeutic plasma exchange (TPE) is plasmapheresis where the removed plasma is replaced with a substitute. At Next Health, that substitute is sterile albumin. So all TPE is plasmapheresis, but it specifies what goes back in.
No referral is required. Every protocol begins with a consultation and physician review to confirm TPE is appropriate for your goals and health history.
It is the opposite mechanism. Young-plasma approaches add someone else’s plasma. TPE removes your own plasma and its accumulated burden, replacing it with sterile albumin, nothing from another person.
TPE has drawn attention in the performance and longevity world. What matters is whether it is the right procedure for you, confirmed in a consultation, and whether your results are measured. That is the conversation we have in your consultation.
Candidacy is determined by a physician. Some people are not candidates, including those with certain bleeding or clotting disorders, albumin allergy, or hemodynamic instability. The consultation and baseline labs determine candidacy before anything is scheduled.
It depends on your baseline and goals. We offer 3-session and 6-session protocols; the right number is set with your physician using your data, not a fixed script.
TPE has decades of established use for recognized medical indications, and its use for longevity and performance is off-label. Here it follows a physician-designed protocol, performed by a registered nurse in a sealed, sterile, single-use circuit. Like any medical procedure it carries risks. These can include a temporary drop in blood pressure, a reaction to the citrate anticoagulant or to albumin, and issues at the access site. Your provider reviews these with you, and your consultation and baseline labs confirm whether it is appropriate before anything begins.
A baseline test that confirms your current levels and clears you before treatment, a consistent standardized protocol across every location, and an integrated longevity ecosystem around the procedure.
